【中图分类号】R４７【文献标识码】B 【文章编号】１００８－６３１５(２０１５)１０－０３１５－０１神经外科患者因中枢神经系统疾病、创伤、感染、高热引起过度通气,体内CO２排出过多,PaCO２ 降低导致呼吸性碱中毒,面罩吸氧是纠正过度通气的重要方法,临床护理过程中发现,传统的口鼻面罩应用气管切开套管口处时存在封闭不严、不宜固定、改善过度通气效果不理想等缺点,影响使用效果,我科现采用一次性口杯辅助纠正呼吸性碱中毒,取得良好效果,现将方法介绍如下:
１ 材料一次性口杯１个、绷带、纱布及剪刀;２ 制作用剪刀在一次性口杯杯底剪一直径约２．５cm 的圆形小孔,在杯身上１/３处剪一直径约５mm 小孔,杯口处左右两侧对正各剪一直径约５mm 小孔,用绷带系好.具体方法见图[１]３ 使用方法将制作好的一次性口杯利用绷带与颈部气管套管牢固固定于气管套管的马连带上,捆绑松紧度以一次性口杯与气管套管处程９０度为宜.平时纱布覆盖到杯底处,吸痰时从杯底部圆孔处伸进吸痰管进行吸痰操作.
４ 优点(１)封闭性好:传统面罩用于气管切开套管口处四周因封闭性不严,过度换气不能得到明显改善,而一次性口杯能利用绷带和颈部气管套管固定绳(马连带)固定,封闭性好.(２)易固定:传统面罩置于气切口处固定绳易脱落,而一次性口杯用绷带直接系在马连带上固定良好.(３)价格低廉:传统面罩１５元左右,一次性口杯０．５元左右,家属易接受.(４)吸痰方便:传统面罩吸痰时需先将面罩摘下后再行吸痰,而一次性口杯可以直接从杯底部圆孔处进行吸痰操作.Neurosurgerypatientsbecauseofthecentralnervoussystemdisease,trauＧma,infection,highfevercausedbyexcessiveventilation,thebodytoomuchCO２discharge,PaCO２ decreaseleadtorespiratoryalkalosis,oxygenmaskisoneoftheimportantmethodstocorrectexcessiveventilation,foundintheprocessofclinicalnursing,themouthofthetraditionalnasalmaskapplicationtracheostomytubemouthisclosedlax,shouldnotbefixed,improvementtheshortcomings,suchasexcessiveventilationeffectisnotidealeffectuseeffect,Idivisionisnowusingdisposablecupsauxiliarycorrectrespiratoryalkalosis,obtaingoodresult,nowtheintroducedmethodisasfollows:１．Material:disposablecup１,bandages,gauzeandscissors;２．Production:withscissorsondisposablecupscupbottomcutacircularholeisabout２．５cmindiameter,onacupofathirdplacetocutadiameterofabout５mmholes,placeleftandrightsidesatthetopforeachcutadiameterofabout５mmholes,fastenedwithabandage．Specificmethodsasshowninfigure(１)３．Themethodofuse:willhaveaone－timecupusingbandagesandcerviＧcaltracheacannulafirmlyfixedonthetrachealtubehorsejoint,bundlesfirmＧnesswithdisposablecupsandtracheacannulacheng９０degreesisadvisable．Ascendagauzecovercupatordinarytimes,whenthesputumsuctionfromtheroundholeatthebottomofthecupintosputumsuctionofsputumsuctionpipeoperation．４．Advantages:(１)closed:traditionalmaskusedfortracheostomytubeaＧroundthemouthforsealingabilitynotYan,hyperventilationcannotgetimprovedsignificantly,anddisposablecupcanusebandagesandneckT(hetrachealtubefixedrope(joint)isfixed,goodsealingability． ２)isfixed:traditionalmaskinfixedropeiseasytofalloffgasslit,anddisposablemouthA(bandageisonthehorsejointdirectlyfixedwell． ３)thepriceislow:traditionalmask１５yuan,one－timekoubei０．５yuan,iseasilyacceptedbyfamilymembers．Easyandsputumsuction (４):themaskofftraditionalsputumsuctionmaskwhenusingitagainafterthesputumsuction,anddisposablecupcanbedirec
