摘要:目的: 检测UC患者结肠黏膜中ARE的结合活性及HO-1表达的变化, 分析ARE、HO-1的表达与UC病情严重程度的相关性.

方法: 选取UC活动期患者为病例组, 同时期结肠息肉患者为正常对照组. 结肠镜下取直肠-乙状结肠交界处黏膜组织4块. 分别用EMSA法测定ARE结合活性, 免疫组织化学染色方法测定HO-1蛋白表达.

结果: UC各组ARE结合活性均高于对照组, 依次为:重度UC组>轻中度UC组>对照组. UC组结肠黏膜组织中HO-1蛋白表达水平明显高于正常对照组, 依次为:重度UC组>轻中度UC组>对照组。相关性分析:UC患者中ARE结合活性的变化与HO-1表达的变化呈正相关。

结论: 随着溃疡性结肠炎患者病情的进一步加重, 结肠黏膜组织中ARE结合活性不断增强, HO-1表达也不断增加, ARE与HO-1的关系呈正相关, ARE的结合活性及HO-1的表达强度在一定程度上反映UC的病情严重程度.

Antioxidant Response Element Binding Activity and heme oxygenase-1 Expression in Colonic Mucosa of Ulcerative Colitis Patients

AIM:To detect the ARE binding activity and HO-1 expression in colonic mucosa of UC patient and analyze the respectively revelance of ARE, HO-1 with severity of UC.

METHODS: This study collected 42 patients who are diagnosed as active ulcerative colitis as case group and 25 patients with colonic polyps as control group. Colonic mucosal tissue (rectum - sigmoid junction) was biopsied 4 pieces under colonoscope. ARE binding activity was detected by EMSA and the expression of HO-1 was detected by immunohistochemistry staining methods.

RESULTS: The ARE binging activity in UC colonic mucosa is higher than that in the control group, as follows: severe active group > mild - moderate active group > control group. The HO-1 expression in UC colonic mucosa is higher than that in the control group, as follows: severe active group> mild-moderate active group > control group. The change of ARE binging activity and HO-1 expression in UC patients is of positive correlation.

CONCLUSION: With the further enhancement of the UC disease severity, the ARE binging activity in UC patients' colonic mucosa become stronger. The HO-1 expression detected by immunohistochemistry is also increasing. The relationship of ARE and HO-1 is of positive correlation. The ARE binging activity and HO-1 expression reflect the severity of UC in a certain extent.

1、材料和方法

1.1材料

选取2013年11月至2014年11月我院42例UC活动期患者,轻中度22例,重度20例。选取同时期结肠息肉患者25例作为对照组。UC患者依据Sutherland DAI评分、UC病情活动度Truelove和Witts分级分为轻度、中度、重度,并进行评分,均数依次为4.8±0.2分、8.6±0.7分、11.5±0.4分。(对照组=A组,轻中度UC=B组、重度UC=C组)。

1.2方法

1.2.1采用EMSA法[1]检测直肠-乙状结肠交界处[2]黏膜组织中ARE结合活性。用Quantity One分析软件以积分光密度值表示ARE结合活性。

1.2.2.免疫组织化学方法(SP法)测定HO-1的表达[3]:所有染色标本均采用Motic Med 6.0数码医学图像分析系统,以阳性细胞染色的积分光密度值(OD)值来表示HO-1蛋白表达量。

2、统计学分析:

使用SPSS 13.0统计软件进行统计分析,计量资料符合正态分布以均数±标准差行方差分析,非正态分布进行非参数秩和检验。相关性分析用线性相关分析。P<0.05有统计学意义。

3、结果

3.1免疫组织化学(SP)法测定HO-1蛋白表达

HO-1在正常结肠黏膜较少表达,UC患者HO-1表达明显增加,主要位于黏膜及腺体上皮细胞胞浆中。统计分析显示:UC各组HO-1的表达明显高于正常对照组,依次为:重度UC组>轻中度UC组>对照组(0.558±0.023 > 0.305±0.046 >0.149±0.019),任意两组进行比较,差异有统计学意义(P<0.01)(图1、2)。

3.2 EMSA法检测ARE结合活性

UC各组ARE结合活性均高于对照组,依次为:重度UC组>轻中度UC组>对照组(70.687±4.058 > 38.078±3.10 > 16.873±1.068)(P<0.01)。任意两组间比较,差异具有统计学意义(图3、4)。

3.3 ARE结合活性与HO-1蛋白表达相关性分析

UC患者中ARE结合活性的变化与HO-1表达的变化呈正相关,r为0.946,P<0.01。

讨论

UC发病机制与氧化应激有密切关联[4]。UC患者抗氧化蛋白的高表达说明UC与氧化应激的密切关系[5-6]。生理状态下,Keapl与Nrf2相互作用,维持机体内环境稳态[7]。当机体受到有害物质刺激时,Nrf2与ARE结合,激活下游抗氧化基因,并促进其转录[8]。研究发现Nrf2表达随UC氧化应激水平的增强而增加,一定程度上反应UC患者的氧化应激水平[9]。Rushmore[10]为研究ARE与其调控的抗氧化基因的机制提供了新的理论。HO-1是ARE所调控的重要的抗氧化蛋白,参与机体抗氧化应激反应。

为了明确UC患者ARE结合活性、HO-1表达及分别与UC病情严重程度的关系,收集UC活动期及结肠息肉患者的结肠黏膜组织,分别应用EMSA、免疫组织化学染色方法检测ARE活性及HO-1表达情况,结果显示UC患者ARE活性及HO-1蛋白表达均比正常对照组高,并且随着UC严重程度的增强ARE活性和HO-1表达越高,两者呈正相关。考虑在UC活动期,Nrf2与ARE结合,转录激活HO-1表达。因此ARE活性与HO-1表达在UC结肠黏膜组织中较高。本实验入选例数较少,仅用一种实验方法分别对两个指标进行研究不够全面,需进一步证实ARE和HO-1与UC的关系,为UC的发病机制提供更充足、更全面的理论依据。
