【摘要】目的:探讨运动训练联合无创正压通气对COPD合并Ⅱ型呼吸衰竭患者肺康复的疗效。方法:将我院2016年4月到2018年11月的76例COPD合并Ⅱ型呼吸衰竭患者,随机分组,常规治疗组给予无创正压通气,运动训练联合无创正压通气组开展运动训练联合无创正压通气。比较两组疗效;住院的时间;治疗前后患者肺康复指标以及运动耐力;并发症率。结果:运动训练联合无创正压通气组的疗效更高,P<0.05。运动训练联合无创正压通气组的疗效是100%,其中38例均有效,而常规治疗组的疗效是76.32%,其中29例有效。治疗前两组患者肺康复指标以及运动耐力比较,P>0.05;治疗后运动训练联合无创正压通气组患者肺康复指标以及运动耐力优于常规治疗组,P<0.05。运动训练联合无创正压通气组住院的时间优于常规治疗组,P<0.05,运动训练联合无创正压通气组住院的时间是8.58±3.46天。常规治疗组住院的时间是12.55±3.01天。运动训练联合无创正压通气组并发症率更少,P<0.05。运动训练联合无创正压通气组并发症率有1例,而常规治疗组并发症率有8例。结论:COPD合并Ⅱ型呼吸衰竭患者实施运动训练联合无创正压通气效果确切,可有效提高患者的治疗依从性,改善其运动耐力和促进患者康复进程,缩短住院的时间和减少并发症的发生。

【 Abstract 】 objective: to study the sports training joint noninvasive positive pressure ventilation pulmonary rehabilitation in patients with COPD combined Ⅱ type of respiratory failure. Methods: from April 2016 to November 2018, 76 cases of patients with COPD combined Ⅱ type of respiratory failure, random grouping, routine treatment group given noninvasive positive pressure ventilation, joint noninvasive positive pressure ventilation in sports training group to carry out joint noninvasive positive pressure ventilation in sports training. The efficacy of the two groups was compared. Length of hospital stay; Pulmonary rehabilitation index and exercise endurance before and after treatment; Complication rate. Results: the curative effect of exercise training combined with non-invasive positive pressure ventilation group was higher, P < 0.05. The effect of exercise training combined with non-invasive positive pressure ventilation group was 100%, 38 of which were effective, while the effect of conventional treatment group was 76.32%, 29 of which were effective. Before treatment, pulmonary rehabilitation index and exercise endurance were compared between the two groups (P > 0.05). The pulmonary rehabilitation index and exercise endurance of patients in the post-treatment exercise training combined with non-invasive positive pressure ventilation group were better than those in the conventional treatment group (P < 0.05). The hospitalization time of the exercise training combined with non-invasive positive pressure ventilation group was better than that of the conventional treatment group (P < 0.05), and the hospitalization time of the exercise training combined with non-invasive positive pressure ventilation group was 8.58±3.46 days. The length of hospital stay in the conventional treatment group was 12.55±3.01 days. The complication rate of exercise training combined with non-invasive positive pressure ventilation group was lower, P < 0.05. The complication rate of exercise training combined with non-invasive positive pressure ventilation group was 1 case, while that of conventional treatment group was 8 cases. Conclusions: COPD combined Ⅱ patients with respiratory failure to implement the joint effect of noninvasive positive pressure ventilation in sports training, exactly can effectively improve the treatment of patients with compliance, improve their exercise endurance and promote patients recovery process, shorten hospitalization time and reduce the occurrence of complications.

无创正压通气对COPD合并Ⅱ型呼吸衰竭是常见的治疗方案,配合运动训练可改善患者的病情和促进肺康复[1-2]。本研究将我院2016年4月到2018年11月的76例COPD合并Ⅱ型呼吸衰竭患者,随机分组,常规治疗组给予无创正压通气,运动训练联合无创正压通气组开展运动训练联合无创正压通气,比较两组疗效、住院的时间、治疗前后患者肺康复指标以及运动耐力、并发症率,分析了运动训练联合无创正压通气对COPD合并Ⅱ型呼吸衰竭患者肺康复的疗效,具体如下。
1资料和方法
1.1资料
将我院2016年4月到2018年11月的76例COPD合并Ⅱ型呼吸衰竭患者,随机分组,运动训练联合无创正压通气组年龄64-89(75.11±2.22)岁。男18例,女20例。常规治疗组年龄64-88(75.25±2.01)岁。男20例,女18例。两组资料无显著差异。
1.2方法
常规治疗组给予无创正压通气,采取NIPPV通气,经面罩采取单向活瓣螺旋管和无创呼吸机连接,并选择S/T 模式通气,结合患者的情况选择合适鼻罩,维持面罩和颜面之间适当贴紧,不至于漏气。结合患者耐受情况对吸气压力调节直至12~18cmH2O,并维持呼气压力4~6cmH2O,每次治疗两小时,每天三次。
运动训练联合无创正压通气组开展运动训练联合无创正压通气。在无创正压通气的基础上给予患者运动训练,主要包括有氧呼吸操训练、不行训练、呼吸肌肌力和耐力训练等。
1.3观察指标
比较两组疗效(有效:病情改善,且肺功能指标改善50%以上;无效:病情无明显好转甚至恶化);住院的时间;治疗前后患者肺康复指标以及运动耐力;并发症率。
1.4统计学处理
SPSS22.0软件, 数据处理采取t检验和卡方检验;P<0.05表示差异有意义。
2结果
2.1疗效
运动训练联合无创正压通气组的疗效更高,P<0.05。运动训练联合无创正压通气组的疗效是100%,其中38例均有效,而常规治疗组的疗效是76.32%,其中29例有效。
2.2患者肺康复指标以及运动耐力
治疗前两组患者肺康复指标以及运动耐力比较,P>0.05;治疗后运动训练联合无创正压通气组患者肺康复指标以及运动耐力优于常规治疗组,P<0.05。见表1.
2.3住院的时间
运动训练联合无创正压通气组住院的时间优于常规治疗组,P<0.05,运动训练联合无创正压通气组住院的时间是8.58±3.46天。常规治疗组住院的时间是12.55±3.01天。
2.4并发症率
运动训练联合无创正压通气组并发症率更少,P<0.05。运动训练联合无创正压通气组并发症率有1例,而常规治疗组并发症率有8例。
3讨论
在无创正压通气治疗COPD合并Ⅱ型呼吸衰竭临床治疗同时,配合运动训练,可加速康复进程[3-4]。其中,患有慢性呼吸衰竭的COPD患者具有二氧化碳潴留,导致高碳酸血症等,患者常有不同程度的呼吸肌疲劳或疲惫。 无创正压通气治疗COPD患者可减少COPD气流阻塞,改善患者呼吸功能,增加动脉血氧饱和度,改善缺氧,减轻症状。运动训练可有效改善COPD患者的呼吸肌力,减轻呼吸困难,提高运动耐力[5-6]。

综上,COPD合并Ⅱ型呼吸衰竭患者实施运动训练联合无创正压通气效果确切,可有效提高患者的治疗依从性,改善其运动耐力和促进患者康复进程,缩短住院的时间和减少并发症的发生。
